Opt In Workflow

  1. Therapy provider subscribes to azeloapp.com (Azelo, LLC) practice management system

  2. Therapy provider gives paper intake forms to their client

  3. Their client completes the Consent to Communicate by SMS Text/Messaging Form as part of the intake paperwork process

  4. The client signs to provide consent to the therapy provider in order to receive SMS communication from the therapy provider.

  5. Therapy provider gives copies of signed paperwork to the client for their own records.
